Our practice philosophy is to treat each companion animal as the individual they are. This often includes Chinese medical treatments such as herbs and acupuncture. Other modalities such as spinal manipulation, laser therapy, and stem cell therapy are available. We practice truly integrative medical care. Please inform us if you are uncomfortable with this approach.
My signature below serves as agreement to the following: I am responsible for all charges incurred by my pet while under the care of the doctors and staff of Hopewell Animal Hospital and understand that these charges are due and payable at the time of services. Treatment plans requiring comprehensive care estimated at $1000 or more, will require a 50% deposit to begin your pet’s treatment. We accept cash, personal checks, Visa, Mastercard, Discover, American express and Care credit as methods of payment. Returned checks are subject to a fee of $30. Any balance that is carried over a period of 30 days will accrue a monthly finance charge of 1.5% or 18% per annum. Any unpaid balance over 90 days or in the event of a default, will leave you responsible for all reasonable collection fees, attorney’s fees and costs.
